How to set up a lake pollution problem that can be modeled by a second order linear equation?

For my differential equations class, my assignment is to create a lake pollution problem that can be solved as both a second order ODE or as a system of first order ODEs. So my question is, how many inputs/outputs would this lake need to have in order to be a second order equation and not a first order?
Would I need a river bringing in clean water, a river draining the lake, and a source of pollution? Or could I just have a polluted river flowing into the lake and another river draining the lake?
