Creating a university letterhead

I'm newbe at LaTeX and I have an assignment that should be written in a page in this form, but I don't know how to make that header in LaTeX, and I want to write it in LaTeX instead of Word, because I'm already trying to learn LaTeX and its an opportunity to deal with something new in LaTeX,
